How to Order an ATM or a Debit Card
- Log in to online banking or via the mobile app.
- Select “Account Services.”
- Under “Checking Accounts,” select “Request a New Mastercard Debit Card.”
- Follow the prompts to complete your request.
You will receive your card in approximately seven business days. If you need to change the account the card is linked to, please contact Member Services at (800) 874-5544.
Need to dispute a debit card transaction?
Log in to your Quorum account, under the “Account Services” menu select “Transaction Dispute.” Follow the prompts to submit a debit card dispute.
Make Changes to Your Account
Updated Your Address
Log in to your account, go to “Settings,” then select “Profile” to make changes to your address or contact information.
Change Your Name or Edit Joint Owners
Download this form to officially change the name on your account or remove a joint owner from your account.
Add Joint Owners
Download this form to add a joint owner on your account. You’ll need to submit a copy of the joint owner’s valid photo ID with this application.
Add / Edit Beneficiaries
Download this form to add/remove a beneficiary. Once you’ve done this, you can manage your existing beneficiaries in online banking.

Use this form to submit a direct deposit request from your employer or funding source to Quorum.
